Hi Ms Lazin,
Thanks for your call yesterday. I wanted to email you over our new patient paperwork, please send it over so we can confirm your appointment slot. Currently, Dr. Sandhu’s schedule is booked pretty tightly. Please let me know if either of the following times might work for you?
Tuesday 10/22 @ 12p
Monday 12/5 @ 1p
Once we receive the completed paperwork I can also add you to our wait list in case there is a cancellation earlier.
The initial consultation will be 2 hours long so please plan accordingly. Please see the attached new patient paperwork which will need to be filled out and faxed or emailed back to us prior to the appointment (the forms can also be found in the link below). See below for more details:

For new patients, please download and complete the new patient forms from our website prior to your appointment. You may fax them in prior to your appointment to us at(310) 491-5957.
Payment should be rendered at time of service via check, we also accept Visa and Mastercard, please complete the credit card authorization form. We do require a credit card on file for missed appointments, etc. For any questions regarding our cancellation policy, rates, etc. please refer to:
